Thank you to our RN, allied health, and PSW collaborators for their insight and wise advice during our long-term care (LTC) to hospital transitions tool focus groups at Perley Health today. It is inspiring to work together with frontline care providers to plan how we will introduce, test and evaluate our tool that aims to empower LTC residents and care partners in making decisions about whether to seek care in the emergency department or at their LTC home.
