Health service providers and health system planners often have trouble using information from researchers on how to deliver better care. Research is usually focused on understanding and treating a single condition in individuals. But in reality, many conditions are interconnected, and would better be managed when considered together. By interconnected, we mean that the presence of one health condition may lead to development of another or that they may have the same risk factors. In these projects we are working with communities to understand their needs related to brain-heart interconnected conditions and what needs to change to better address these needs. We also want to ensure that others can use what we learn and apply it to other communities to improve care for individuals with brain-heart interconnected conditions.
