Optimizing Regional Care Delivery Systems Targeting Adults with Heart Failure and Mental Health Interconnected Conditions: A Participatory Action Research Brain-Heart Dyad Project.

We are collaborating with the Archipel and Great River Ontario Health Teams to support them in leveraging existing knowledge to improve the care they deliver for a pair of interconnected brain and heart diseases relevant to their communities, specifically heart failure and mental health conditions including depression. We have brought together key individuals from these communities to work together to make decisions about the direction of this project. In the first year, we will consult broadly to understand each community’s needs related to the brain-heart dyadic conditions and what needs to change to better address these needs. We will then review scientific evidence to identify what changes we can make to improve care delivery for these conditions at a systems level and produce a plan for the changes the community selects to make. We also want to ensure that others can use what we learn and apply it to other communities to improve care for individuals with heart failure experiencing mental health issues.
